New tool allows you to access your health information

Sarasota Memorial’s Patient Portal is here!
It’s launch time for the new online tool that will help you manage your protected health information in one secure, accessible place. If you’ve ever been an SMH patient, we invite you to be among the first to establish your SMH FollowMyHealth portal account at a special event at HealthSquare at Westfield Sarasota Square Mall on Monday, July 7, from noon to 3pm.

Just bring a valid photo ID (current driver’s license, passport or state/federally issued ID) to HealthSquare and we’ll get you registered. It’s free, easy and it’s your permanent, secure portal to all your future SMH health information.

Benefits of signing up include:

• Check personal medical information anytime, 24/7
• View test and lab results for future visits at SMH facilities
• Set up a proxy account for children and dependent adults
• More functionality to come as upgrades are completed!

I have received two separate invitations and tried to sign up. When I get to the code page, I tried the last four of my social and was denied. I also tried the year of my birth and was denied. There is no help page, so no recourse, so I am left in limbo.

I had the same problem as Jane — would not recognize my SSN, although it allowed me to create a username and certain profile info. I’ve also successfully changed my password, but can’t get in wit either.

I have a log in name and password but cannot easily find the place to log in. Mostly I land on the page to set up a new account, which I already have.

And, when I have found the correct log-in page, I cannot leave it to see my information. It just locks on the name/password page with the green lock logo. Even changing my password cannot get me beyond that page, although I can see that it recognizes me.
